Procedure for Requesting Accommodations

  1. Fill Out the Application

Register in the Accommodation Information Management (AIM) system. Complete the Housing/Meal Plan Accommodation Application. (Despite the title, this form is also used for emotional support and service animal accommodations.)

  2. Upload Documentation

    Housing/Meal Plan Accommodation Provider Form

Ask your provider to complete the Housing/Meal Plan Accommodation Provider Form or write a letter to address your request. If your provider prefers to write a letter, the document should include the disability/medical/mental health diagnosis and detailed accommodation recommendations. Those recommendations should focus on necessary environmental or dietary needs, not specific locations. For example, a provider might indicate you need a location where you can access a kitchen, not that you need housing in specific campus apartments.

It is rare that Bon Appétit is unable to meet a student's needs and DAS approves a full exemption to the meal plan. In some instances a reduced meal plan is suitable. With that said, each application is looked at individually, recognizing unique circumstances for each student. Students are always welcome to connect with Bon Appétit to explore available options for specific dietary needs. Contact them at (503-554-3205) or bamgr@georgefox.edu

NOTE: DAS cannot make a determination until documentation is submitted from an appropriate provider.

The Housing Application can be completed a single time by students requesting more than one type of housing or meal plan accommodation. See the categories above for the documentation needed for each type of accommodation, and submit with your application. Please note that provider documentation that is applicable to one accommodation request may not be sufficient to support a different request.

  3. Meet with DAS Staff

Schedule a meeting with Disability & Accessibility Services (DAS) over Zoom or in person. DAS will discuss if more information is needed. Please note that everyone requesting an ESA in campus housing must have an interview with DAS.
  4. Contact the Office of University Housing

Upon approval of your housing request by Disability & Accessibility Services, communicate with the Office of University Housing (housing@georgefox.edu) to discuss specific accommodation arrangements.

Upon approval of ESA through DAS, you MUST receive housing approval before moving your ESA into your room or you risk fines from housing
